We completely respect your privacy. Use of PainApp does not require you to enter your name, social security number, medical record, or date of birth. We intentionally designed the tool so that it could be used in a way that protects your confidentiality.

If you choose to email the summary report to yourself after viewing the tool, you will need to enter an email address.But that is your choice. If you do not want to enter your email address, you can simply take a screenshot of your personal summary when you view the tool.

We include a short evaluation of PainApp and give you an opportunity to give us feedback to improve the tool. Findings from this evaluation may be used anonymously, in aggregate, without identifying the identity of any one person. The tool does not ask you to enter your name so you can rest assured that your identity will be protected.
